Initiate Your Request
Log into your FormDTM account and navigate to your order history to select the items you wish to return or exchange.
Prepare the Package
Place the items in their original packaging with all tags attached to ensure they remain in brand-new, resalable condition.
Ship the Items
Affix the prepaid shipping label provided via email to your parcel and drop it off at any authorized carrier location.
Receive Your Refund
Once our warehouse inspects the returned goods, your refund or exchange will be processed within 5 to 7 business days.
